#screening
Early detection of #congenital_heart_diseases is one of the challenges that still persists.
The reason is that in some cases, the baby has no clinical symptoms at birth or shortly after, and nothing about #CHD has been found in prenatal screenings.
The solution proposed by the texts is to screen all newborns by pulse oximetry before discharge.
After 24 hours from the birth of the baby (preferably on the second day of life), the baby is screened by pulse oximetry.
The way to perform the procedure is that in both pre-ductal (right hand) and post-ductal (one of the legs) organs, pulse oximetry is performed simultaneously or alternately.
Positive screening criteria:
1. Arterial oxygen saturation (SpO2) should be less than 90%.
2⃣ SpO2 level less than 95% in both pre-ductal and post-ductal organs in three measurement times at an hour interval.
3⃣ SpO2 difference of more than 3% between the pre-ductal and post-ductal organs.
Through this screening, seven common #CHDs are identified:
#HLHS
Hypoplastic left heart syndrome
#Pulmonary_atresia (PA)
Lung atresia
#TOF
Tetralogy of Fallow
#TAPVC
Aberrant pulmonary venous return anomaly
#TGA
Displacement of great vessels
#Tricuspid_atresia (TA)
Tricuspid atresia
#Truncus_arteriosus (TAC)
Common trunk arteriosus
:
An important limitation of pulse oximetry screening is its inability to detect some left heart abnormalities and non-cyanotic cardiac abnormalities such as #VSD.
Health care providers and families should know that the negative results of pulse oximetry screening do not mean rejecting all heart abnormalities!
The universal screening program with pulse oximetry has another advantage, which is the identification of non-cardiac disorders associated with decreased arterial oxygen saturation in term infants. Among these diseases are:
#Pneumonia
#Sepsis
#pulmonary hypertension
#Aspiration of meconium
#Pneumothorax
#Hemoglobinopathy
pointed out Therefore, it is recommended to screen with pulse oximetry in all infants before discharge.
