In general, there are two categories of methods for measuring microalbumin:
1- Methods based on the use of colors such as Komusi Blue
2- Immunological methods that usually use a monoclonal antibody against albumin and are used in the form of turbidometry or chromatography.
Usually, methods based on color combination lack the sensitivity and specificity necessary for quantitative measurement and are mostly used in screening methods.
Clinical importance:
At first, the existence of microalbuminuria in diabetic patients was investigated and it is said that the probability of chronic renal failure is higher in diabetic patients whose microalbuminuria is more than normal.
Now, there is this idea about other chronic diseases such as high blood pressure, while according to some studies, the possibility of cardiovascular diseases is more in those who have microalbuminuria.
To prove microalbuminuria, 2-3 times of testing should be done within 6 months and their results should be higher than normal.
In this case, these patients are under more care, and of course some treatment measures have been recommended to prevent kidney failure.
For whom is the microalbumin test requested?
According to the recommendation of the National Kidney Foundation in America, all diabetic patients between the ages of 12 and 70 should be checked for microalbuminuria at least once a year.
But the American Diabetes Association says that in type 1 diabetes, 5 years after diagnosis, this test should be done annually, and in type 2 diabetes at the beginning of diagnosis, and then repeated annually.
Hypertensive patients should also be regularly checked for microalbuminuria.
Other cases that increase albumin in urine:
•- Exercise
Urinary tract infection
• Vaginal discharge
• Dehydration
•- The presence of blood in the urine
•- Fever
• Heart failure
Some medicines such as aspirin, glucocorticosteroids and antibiotics
Important point: In order to prevent false positive cases, keep the above points in mind.
Example:
This test is performed in a random sample, a morning sample, a two-hour sample or a 24-hour sample. It is recommended that it be done in the morning sample at the same time as urine creatinine measurement.
The ratio of urinary albumin to creatinine shows a better estimation of albumin excretion status.
Normal values:
•-Less than 30 milligrams of albumin per gram of urine creatinine
